We are pleased to announce that the Adult Care Facility/Assisted Living Residence Common Application has now been posted to the Health Commerce System (HCS). The Department, NY ALFA, and other associations are hosting a number of training sessions across the State to educate DOH staff, providers, and other interested parties on the use of the new application as well as several new forms designed to expedite the review of applications meeting certain specific criteria.
The training session schedule is below:
September 10: Albany
September 23: Syracuse
October 2: Long Island
October 20: Buffalo
October 21: Rochester
November 6: Queens
November 10: Westchester
